StaRxCare operations are mainly centered around pharmacist services supported by and leveraging on provider and other health care services. Pharmacist-evaluated cost savings to the American health system were estimated to be over 2 million USD in one study.¹ A number of other studies in various other settings have validated these findings.^2-9

95.3% of patients in another study stated that their overall health and wellbeing had improved because of pharmacist-provided Medication Therapy Management (MTM) services.¹⁰This is a repeated theme in most healthcare systems where pharmacists provide clinical services, and is the evidence-based premise on which we operate.

Partnerships make our world go round and optimize health outcomes. Our team consists of highly collaborative health care professionals of various fields (pharmacist, physicians, pharmacy technicains, advanced nurse practitioners, nurse care managers, patient services representatives, health coaches, etc.). We partner with and serve individual patients, health plans and various value-basedorganizations such as Independent Physician Organizations (IPOs), Accountable CareOrganizations (ACOs) and Managed Care Organizations (MCOs) to achieve their desired goals. Our core services are summarized below.

Disease & Chronic Condition Management

Integrating distinct but interrelated services is key to optimizing health care efficiency and outcomes. With Disease Management, we optimize outcomes and target indices for commonly encountered disease states (e.g., hypertension & diabetes) while incorporating Medication Therapy Management & Targeted or Comprehensive Medication Reviews. Remote Patient & Clinical Outcomes Monitoring are integral to this. With Chronic Condition Management, we leverage the same to improve & prevent medical conditions in patients with two or more chronic conditions (or as further defined by CMS).

High Risk Medication Management

We assess all medications being taken (including supplements and OTC drugs) for safety, efficacy and appropriateness, and if unavoidable risks are present, we employ strategies to mitigate those risks, especially in patients 65 and older.

Medication Access & Behavioral Health Integration

We work hard to ensure you get your prescribed medications in a timely and hassle-free manner, while facilitating affordability for all (Medication Access & Cost-Containment through Patient Assistance Programs, Coupons, & Formulary Management).

Additionally, we provide access to high quality behavioral health medication management. The application of evidence-based integrated care models allows behavioral health clinicians to be embedded in and become regular and routine contributors to primary care teams. This includes our board-certified psychiatric pharmacists who work in close collaboration with psychiatrists to manage medications within an integrated primary care Behavioral Health program.

HEDIS/STARS Measures

We have a proven data-driven track record of raising HEDIS/STARS medication-related measures from substandard to desirable points (4 or 5 stars). Examples of the measures we impact are all the Medication Adherence, Care of Older Adults (COA), Statin Use in Persons with Diabetes (SUPD), etc.

After-hours Care

We provide After-hours Care including visits with our Physicians and other Providers and Pharmacists to prevent Urgent or Emergency Room visits, worsened health, and unnecessary health care expenses, while facilitating optimal health outcomes, consistency of care and optimized transitions/follow-up. Among others, this also contributes to CMS requirements for Chronic Condition Management.

Global Pharmaceutical Care

We partner with global health organizations to facilitate Medication Access in low access regions, create Pharmaceutical Care Models for Global & Region-Specific populations, and provide Pharmacotherapy & Disease Management Training Programs, etc.
